Job Summary
Elite Care Home Health and Hospice is seeking a Registered Nurse to join our team. This is a per-visit position, ideal for an RN who wants control over their schedule while still doing meaningful, hands-on patient care. You will create and manage individualized plans of care for home health and hospice patients, working independently in patients' homes while staying connected to a supportive clinical team. You are detail oriented with a big heart, take pride in personalized care planning, and communicate effectively in your patient's best interest.
Responsibilities
- Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans for home health and hospice patients
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient condition and report findings promptly
- Maintain accurate, timely records of patient care and discuss observations with your supervisor
- Accept PRN visits based on your own availability
Qualifications
- Currently licensed Registered Nurse (RN) in the state of Texas
- Previous home health or hospice experience preferred
- Excellent interpersonal and patient care skills
- Valid driver's license and reliable transportation for home visits
- First aid/CPR certification required
Position Details
- Fulltime/ PRN, paid per visit
- Serving the Austin, Texas area

Compensation: $38.00 - $50.00 per hour
